Dr. Lachot created his Scottsdale practice with the idea of providing state-of-the-art dental services in a relaxing and comfortable setting. He wanted to create an atmosphere of trust, a place that you and your family members of all ages could look forward to visiting. Dr. Lachot chose his dental support team carefully - in particular, the experience and expertise of Dr. Loo.

In keeping with Dr. Lachot's vision of a high-tech/high-touch practice, we are always studying the latest dental breakthroughs and technology to provide you with the very best treatment options, in the most comfortable of settings.

We care about your dental health, and we want your visit to be as enjoyable as possible. We look at each dental visit an investment in your smile. Whatever you think we can do to further improve your experience, please share it with our staff members.

INSURANCE

We are pleased to submit insurance claims for you. Please bring your insurance identification card to your first dental visit. Payment is expected at the time of service, and you will be responsible to pay the portion not covered by your insurance company at the time of that visit. We will estimate your portion as closely as possible by confirming your eligibility and coverage. Ultimately you are responsible to know and understand your dental plan, and any services not covered by your plan.

HMO, PPO, DMO

We are contracted with Delta Dental Insurance, however patients outside of the Delta Dental Network have chosen this practice for the outstanding dental care that we provide. Some patients, even though they may be enlisted in another provider network, may elect to go outside of their network for dental services. You may be eligible to see us for your dental care, but be sure to contact your employer's human resource coordinator to verify coverage

FEES

In the likely event that your insurance company does not pay your dental fee in full, the patient is responsible for any balance accrued on their account. We will make every attempt to maximize your insurance coverage, however there are occasions when an insurance company will not cover a service for various reasons, especially those that are cosmetic in nature. We also provide patients with a written and verbal estimate prior to beginning any treatment.

PRE-DETERMINATIONS

With your permission, a written pre-determination may be sent to your insurance company to verify coverage and to give a more accurate estimate of your patient portion. However, a pre-determination is not a guarantee of insurance payment; it is only an estimate.

FINANCING

For your convenience, outside financing options are available through Care Credit. Options include 12 months, same as cash, or financing up to 60 months, upon credit approval. You may be approved in as little as 15 minutes. Let us know if you would like to take advantage of this financing service.

 

CANCELLATION POLICY

As a courtesy, we call to confirm scheduled appointments two days in advance. We require 48-hour notice to cancel or change an appointment. A fee of $75 may be charged for broken appointments. Our office hours are Monday through Thursday, 8 am to 5pm. If we are not in the office or are unavailable at the time of your call, a message may be left on the voice mail.

We understand that there are times when there is a sudden illness or an extenuating circumstance occurs, however multiple last minute cancellations will be treated as a missed appointment and a fee may be charged.
